I vaguely remember from my druid's epic flight form quest that the Nightmare is actually caused by the Raven God, Anzu. Weird that none of that is in the WoWWiki article on the Nightmare.

Anzu isn't *the* cause of the Nightmare, but he has helped it along quite a bit.
